Forms are ad-hoc checklists that can be started and submitted at any time from the Checklists and Forms page.
Starting a Form
The steps to start a form depends on whether the form has already been submitted on the current day.
If no submissions exist today, selecting the form automatically opens a new submission.
Start a Form (no prior submissions)
Navigate to the Checklists and Forms page.
Select Forms.
If prompted, select the desired location.
Select the desired form.


If at least one submission exists for the form today, selecting the form opens the most recent submission. To initiate a new submission, select Start New from the menu.
Start a New Form (after prior submissions)
Navigate to the Checklists and Forms page.
Select the Forms tab.
If prompted, select the desired location.
Select the menu button for the desired form.
Select Start New.
Each form supports only one incomplete submission at a time. Starting a new form clears any saved entries in the existing incomplete submission.



Complete and Submit a Form
Once a form has been started, any changes are automatically saved. If closed, the form can be reopened and edited until the submit button is clicked.
Complete and Submit an Open Form
Open the desired form.
Expand the form sections to view input fields.
Complete all required fields.
Select Submit.

Reopen a Saved Form
Navigate to the Checklists and Forms page.
Select the Forms tab.
If prompted, select the desired location.
Select the menu button for the desired form.
Select View History.
Select the incomplete form.
