1099-MISC Printable Form

The 1099-MISC Printable Form is produced to report any payments made to a person or organization that were at least $600, or at least $10 in royalties or broker payments. The information on this form is organized so that it can be printed directly onto a 1099-MISC Form and then sent to both the respective person/organization(s) and the IRS for tax purposes. 


Important Note on Printing

When printing this form, ensure to update the following printer settings:

Adobe

When printing from Adobe, the 'Page Sizing & Handling' should be set to 'Actual Size.'

Chrome

When printing from Chrome, 'Scale' should be set to 'Default.'


Navigation

The My Reports search bar can be used to search R365's entire catalog of reports.

  1. Open the Reports app. 
  2. Navigate to My Reports
  3. Enter all or part of the report name in the search bar.
  4. The Results tab will open with the list of search results.
  5. From beneath the report name, click Run to run the report with the selected report view.

    Click Customize to adjust the report parameters and run the report. 



Report Parameters

Field/Column

Description

Year

The year the form is run for. Default: last year.

Legal Entity

A listing of all legal entities that can be selected.

Vendor

A listing of all 1099 vendors associated with the chosen legal entity.

Common Payer

This will run the legal entity as a common payer as long as the legal entity is also marked as a common payer on its legal entity record.

Threshold Amount

The minimum payment threshold for vendors. The default amount is set to $10.

Report Details

Field/Column

Description

Payer's Name and Address

The legal entity's name and address.

Payer's TIN

The legal entity's taxpayer identification number.

Recipient's TIN

The vendor's taxpayer identification number.

Recipient's Name

The vendor name.

Recipient's Street Address

The vendor's street address.

Recipient's City, State, Zip Code

The vendor's city, state, and zip code.

Box 1 Rents

Reports rental income, such as payments for office space, equipment rentals, or land use, made to the recipient.

Box 2 Royalties

Reports royalty payments of $10 or more, including those for copyrights, patents, and natural resources.

Box 3 Other Income

Reports income not reportable elsewhere on the form (e.g., prizes, awards, or punitive damages).

Box 4 Federal Income Tax Withheld

Reports backup withholding or federal income tax withheld due to incorrect taxpayer information.

Box 5 Fishing Boat Proceeds

Reports a crew member’s share of proceeds from the sale of catch on a fishing boat with fewer than 10 crew members.

Box 6 Medical and Health Care Payments

Reports payments of $600 or more made to providers of medical or health care services (doctors, clinics, etc.).

Box 7 Direct Sales Over $5,000

Indicates the payer made direct sales totaling $5,000 or more of consumer products to the recipient for resale. No dollar amount shown—just a checkbox.

Box 8 Substitute Payments in Lieu of Dividends or Interest

Reports substitute payments made instead of dividends or interest, typically under a securities lending arrangement.

Box 9 Crop Insurance Proceeds

Reports proceeds of $600 or more paid to farmers by insurance companies for crop losses.

Box 10 Gross Proceeds Paid to an Attorney

Reports payments to attorneys of $600 or more for legal services, including gross proceeds from legal settlements.

Box 11 Fish Purchased for Resale

Reports amounts paid to sellers of fish or other aquatic life for resale purposes.

Box 12 Section 409A

Reports deferrals under a nonqualified deferred compensation plan subject to Section 409A requirements.

Box 13 Excess Golden Parachute Payments

Reports the portion of a golden parachute payment subject to a 20% excise tax, usually due to its excessiveness.

Nonqualified Deferred Compensation

Reports income from nonqualified deferred compensation plans that doesn’t meet the strict r
