# Donations Form

The Donations Form is a form export that can be printed and completed in the restraurant. Donated items can be tracked and logged to be added to R365 at a later time.

## Navigation

The My Reports search bar can be used to search R365's entire catalog of reports.

1. Open the **Reports** app.
2. Navigate to **My Reports**.
3. Enter all or part of the report name in the **search bar**.
4. The **Results** tab will open with the list of search results.
5. From beneath the report name, click **Run** to run the report with the selected report view. -OR- Click **Customize** to adjust the report parameters and run the report.

## Form Details

| **Field/Column** | **Description** |
| --- | --- |
| **Item** | The name of the item. |
| **U of M** | The unit of measure associated with the item. |
| **Qty** | The amount of the item that was donated. |
| **Comment** | Any comments that detail the reason the item was donated. |
